When is abdominal pain a medical emergency?

Most stomach aches settle on their own, but some patterns need emergency assessment. Sudden, severe, or steadily worsening pain, a belly that is rigid, swollen, or extremely tender to touch, or pain so intense you cannot find a comfortable position can indicate appendicitis, a blocked or perforated bowel, gallbladder or pancreas problems, or internal bleeding.

Go to the emergency room if abdominal pain comes with vomiting blood or material that looks like coffee grounds, black or bloody stools, a high fever, inability to pass stool or gas, a tender pulsating mass, or fainting. Pain low on the right side that started near the navel suggests possible appendicitis. In anyone who could be pregnant, severe lower abdominal pain with bleeding or dizziness needs immediate care to rule out an ectopic pregnancy.

Mild, short-lived cramps from indigestion or a stomach bug can often be managed at home or discussed with a pharmacist. When pain is severe, persistent, or paired with any red flag above, seek urgent in-person care.
