What causes neck pain and stiffness?

Most neck pain comes from muscle strain, poor posture, sleeping awkwardly, or wear-related changes, and settles within days to weeks. Neck pain with fever, severe headache, or arm weakness needs urgent assessment.

Everyday neck pain is usually mechanical, caused by muscle and ligament strain from posture, prolonged screen or phone use, sleeping in an awkward position, or minor injury. It often eases within a few days to a couple of weeks with gentle movement, posture changes, and over-the-counter pain relief. Keeping the neck moving gently, rather than keeping it rigid, generally helps recovery.

Wear-related changes in the neck (cervical spondylosis) become more common with age and can cause stiffness and occasional nerve-related arm symptoms. Whiplash after a car accident can cause delayed neck pain and stiffness.

Seek urgent care if neck stiffness comes with a high fever, severe headache, sensitivity to light, or a rash, as this can suggest meningitis. Also seek prompt assessment for neck pain following significant trauma, or with weakness, numbness, or pins and needles in the arms or hands, problems with coordination or walking, or loss of bladder or bowel control.
