Paracetamol (called acetaminophen in the US and Canada) has a long track record and is widely regarded by health authorities as the preferred option for pain or fever during pregnancy when something is needed. It is used across all trimesters, but the guidance is always to take the smallest amount that helps, for the shortest time, rather than routinely.
Fever itself in early pregnancy can be a concern, so treating a genuine fever may be beneficial. That said, recent research discussions have raised questions about frequent or prolonged use, which is one more reason not to use it casually or long-term without advice.
Do not combine multiple products that each contain paracetamol (some cold and flu remedies do), and speak to a pharmacist before using any combination cold remedy in pregnancy. If you have pain or fever that is severe, persistent, or comes with other symptoms, contact your doctor rather than self-treating.
