What it is used for

  • Scabies (mite infestation of the skin)
  • Head lice
  • Pubic lice

How it works

Permethrin overexcites the nervous system of mites and lice, paralysing and killing them on contact. It stays on the skin or hair to act on the parasites while sparing human nerves.

How it is taken

Applied to the skin as a cream for scabies (usually over the whole body from the neck down, left on for a set time then washed off) or as a cream rinse or lotion for lice in the hair. It is for external use only and a second application is often advised. Use it exactly as your doctor, pharmacist, or the label directs.

No doses here on purpose. The right strength and schedule depend on you — only a doctor or pharmacist should set them. Always follow your prescription and the label.

Common side effects

  • Mild burning, stinging, or tingling where applied
  • Itching (which can continue for a while even after the mites are gone)
  • Redness or mild skin irritation
  • Temporary numbness or rash

Serious effects — get help

  • Signs of a severe allergic reaction such as facial swelling, widespread rash, or breathing difficulty — seek emergency care
  • Severe skin irritation, blistering, or worsening rash — contact your clinician
  • Swelling and oozing suggesting a skin infection — seek care

If a reaction is severe — for example trouble breathing, swelling of the face or throat, or collapse — go to the nearest emergency room now or call your local emergency number. Do not wait for an online consultation.

Interactions to watch

  • Few interactions because it is used on the skin, but avoid applying other skin products at the same time unless advised
  • Tell your clinician about other topical treatments you use

Who should take care

  • For external use only — keep away from the eyes, inside of the mouth, and nose
  • For scabies, wash bedding and clothing and treat close contacts at the same time
  • Itching can persist for weeks after successful treatment and does not always mean it failed
  • Tell your clinician if pregnant or breastfeeding
  • Use the correct strength and method for scabies versus lice — they differ

Frequently asked questions

Why am I still itchy after treatment?
Itching can continue for two to four weeks after the mites are killed because of the skin's reaction. This does not necessarily mean it failed, but check with your clinician if new burrows or spots appear.
Do I need to treat my family?
For scabies, close contacts and household members are usually treated at the same time, even without symptoms, plus washing bedding and clothes. Ask your clinician.
How long do I leave the cream on?
Scabies cream is left on for a set period and then washed off, while lice products differ. Follow the exact time on the label or from your pharmacist.
Do I need a second application?
A repeat application is often advised after a set interval to catch newly hatched parasites. Follow the instructions you are given.
Is Permethrin safe to take?
For most people it is safe when taken exactly as a doctor or pharmacist advises and as printed on the label. Tell your clinician about your other medicines and health conditions, and seek urgent care for any serious reaction.
