Most back pain is muscular and improves within a few weeks with gentle activity and simple pain relief. However, certain red flags point to a serious cause needing urgent care. The most important is cauda equina syndrome: back pain with numbness around the genitals, buttocks, or inner thighs (saddle area), difficulty passing or controlling urine, loss of bowel control, or new weakness or numbness in both legs. This is a surgical emergency to prevent permanent damage.

Other warning signs include back pain following a significant fall or accident, pain with fever or unexplained weight loss, pain that is severe and constant or worse at night, and back pain in people with cancer, a weakened immune system, or osteoporosis, which can suggest infection, fracture, or spread of disease.

Ordinary back pain without these features can usually be managed at home or with advice from a physiotherapist, pharmacist, or doctor. If any red flag appears, particularly bladder, bowel, or saddle symptoms, seek emergency care immediately.
