Does an enlarged prostate always need treatment?

No. A mildly enlarged prostate with few symptoms can simply be monitored. Treatment is considered when symptoms bother you or cause complications, and ranges from lifestyle measures to medication or, in some cases, a procedure.

Benign prostatic hyperplasia (BPH) is so common with age that having an enlarged prostate is often normal and does not automatically require treatment. The decision is driven by how much the symptoms affect your quality of life and whether complications develop, not by size alone.

For mild, non-bothersome symptoms, watchful waiting with lifestyle adjustments, such as reducing evening fluids, caffeine, and alcohol, and bladder habits, is reasonable. When symptoms are more troublesome, doctors may prescribe medications that relax the prostate or shrink it over time. For severe symptoms, repeated urine infections, retention, or kidney effects, a procedure or surgery may be recommended.

See a doctor to assess your symptoms and rule out other causes. Seek urgent care if you suddenly cannot pass urine, have blood in your urine, or develop fever with urinary symptoms, as these signal complications that need prompt attention.
